Our flat fee includes drafting the following forms, making 2 rounds of changes, and discussing them with you. This process usually takes 4-6 weeks. Please note, we customize the package for each practice’s needs, so these documents might be combined and/or reorganized based on what works for you:

- Consent for Payment (insurance, cancellation policy, late arrival policy, type of payment accepted, late payment policy)

- Good Faith Estimate Template (for self-paying clients)

Over the years, we have helped a variety of health and wellness practitioners develop their client forms and we know that the services vary significantly between practitioners. Sometimes additional forms are necessary to lay out the specific risks, applicable laws, and practitioner policies for certain services or clients. For such situations, we can offer additional forms, such as:
- Consent for Minors
- Consent for Recordings/Photos Taken and Consent to Use Recordings/Videos
- Consent for Specific Treatments, e.g., aquatic therapy, in-person therapy outside of the office, functional behavior assessments, or treatments that involve physical contact to clearly disclose the risks and benefits
